RELEASE FOR EMPOWERMENT XIV DEFENSE SYSTEM
The undersigned hereby acknowledges to Empowerment XIV and its Founder, Executive Board, Staff and Instructor(s) that she/he will not participate in any aspect of the program that she/he is uncomfortable with or considers unsafe. That should they choose to participate is aware of the physical nature and possible risks of injury incident to taking this practical course in self defense. That she/he is physically fit to participate in this course, involving various physical techniques, and realizes that self defense techniques cannot be successfully employed in every situation. Proficiency can only be achieved and is dependent upon thorough continued practice, exercising good judgment, and a person’s natural abilities. The undersigned hereby releases Empowerment XIV, its Founder, Executive Board, Staff and Instructor(s), and agrees to hold them harmless, from any liability for injury that may be incurred as a result of participation in this course, or using the strategies within for defense. The undersigned also acknowledges that Evolution Self Defense is not responsible for the selection of trainers, training environments, training procedures or training equipment that an individual instructor may use during this program.
I HAVE READ THE ABOVE WAIVER AND RELEASE, UNDERSTAND THAT I GIVE UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND I SIGN IT VOLUNTARILY.
In consideration for my child’s attendance and participation in the self defense training offered by Evolution Self Defense, I acknowledge the existence of certain inherent risks in this type of training and hereby agree to assume all risks. As guardian, I further relieve the school, its management, assigned staff, and fellow students from liability resulting from loss, whether personal belongings or bodily injury. I also hereby state that he/she is physically fit to take the prescribed course of instruction and does so of his/her own free will.
